供覧症例は、子宮体癌IA期、類内膜腺癌G1で開腹単純子宮全摘+両側付属器 切除+骨盤リンパ節生検を行った（2008年当時）。見どころは、膀胱剥離、尿管剥離、ダグラス窩腹膜切開を鋭的に行うところ、筋膜外子宮摘出を腹膜下で行うところ（＝腹膜を巻き込まないで傍子宮組織を挟鉗・切断・結紮）です。膀胱や直腸を牽引の方向を工夫して、剥離すべき層を“面”として見つける展開法が随所に示されています。

Abdominal Simple Hysterectomy

The patient presented with stage IA uterine cancer and G1 endometrial adenocarcinoma,
who underwent open simple hysterectomy + bilateral adnexectomy + pelvic lymph node biopsy (at the time of 2008). Resection of the uterus,
bilateral adnexa, and pelvic lymph node biopsy were performed. Highlights are the sharp bladder dissection, ureteral dissection, Douglas fossa peritoneal incision, and extrafascial hysterectomy performed subperitoneally (i.e., pinching, cutting, and ligating the paraspinal uterine tissue without involving the peritoneum).
The method of deployment is illustrated throughout, in which the bladder and rectum are deployed in different directions of traction and the layers to be dissected are found as “planes”. Audio and text are in Japanese.

症例は子宮頸がんIB１期扁平上皮癌です。動画は基靭帯処理の後から始まります。 予め、尿管を包む膜（腎筋膜前葉（尿管下腹神経筋膜））の外側を走行し基靭帯断端に合流する血管束を一括処理し、赤色の血管テープで縛ります。 次に通常の膀胱子宮靭帯前層を処理する術野に戻し、尿管を転がしていくと、テープに巻かれた血管束は膀胱子宮靭帯後層を処理する血管でした。以上より、膀胱子宮靭帯後層処理では、膀胱静脈を尿管下腹神経筋膜の外側で結紮していることがわかりました。

TThe patient has stage IB1 squamous cell carcinoma of the cervix.
The video begins after the cardinal ligament process.
The bundle of vessels that ran along the outside of the anterior renal fascia (UHF)
and merged into the cardinal ligament transection was collectively ligated and tied with yellow vessel tape.
Next, we returned to the surgical field where the anterior layer of
the vesicouterine ligament was usually treated and rolled the ureter.
The vascular bundle tied with tape identified the cephalic resection stumps of the posterior layer of the vesicouterine ligament.
