JOS members should notify their membership ID numbers at pre-registration.
<Staff and Graduate students>
Graduate students are required to present ID cards whether they have pre-registered or register on site.
Dentistry staff members are required to submit certificates issued by their dentistry office or the JOS 71st Annual Meeting form (download from here) signed by the head of their dentistry office.

Contact the Secretariat in writing [by Fax or mail]. Cancellation will be effective on the day the Secretariat receives the notification.
A full refund will be given for cancellations made by Friday, 20 July 2012. (Refund will be made after the Meeting.) No refund will be available for cancellations made past this date.
