Accommodation Application Procedures
General Information
JTB Tohoku Corp. (JTB) has been appointed as the official travel agent for The 71st Annual Meeting of the Japanese Orthodontic Society and will arrange hotel accommodation. JTB has reserved a sufficient number of rooms at the following hotels for the participants during the meeting period.
Hotel assignment will be made on a first-come, first-served basis.
Hotel Information
The following hotels have been reserved for the convenience of participants.
*Room charges are valid for the stay from Sep 25 to Sep 28, 2012 (4 nights).
*Room charges are per person, per night, including breakfast, service charge and consumption tax.

*No Tour Leaders will be receiving the guests at the airport. All guests are required to make own arrangements to the hotel for check in.

Reservation for accommodation has been closed.
Payment Method
Please pay all charges by credit card. In order to make a hotel reservation, a full deposit is required.

Confirmation
*A confirmation of your reservation and map will be sent afterwards.
Cancellation / Revision
*Inform JTB of any changes in your dates of arrival and/or departure or of cancellation.
*In case of cancellation, your deposit will be refunded after deducting the cancellation fees as shown below.
*Cancellation fee when notice is given:

*You MUST give notice to JTB if you intend to check in at the hotel later than 21:00 on the intended arrival date, or your reservation may be canceled.
