月経前不快気分障害：T.A. Grady-Weliky. Premenstrual Dysphoric Disorder. N Engl J Med 2003;348:433.
月経前不快気分障害は重症型の月経前症候群であり，生殖年齢の女性の 3〜8%にみられる． 33 歳の女性は，月経開始 1 週間前から始まり月経開始後 3 日以内に消失する，苛立ち，号泣，気分のむら，ひどい倦怠感を報告している．症状は，患者の通常の社会生活を妨げ始めている．医師は，この患者をどのよう に治療すべきであろうか？

体重減少原因の検索：Unintentional weight loss: diagnosis and prognosis. The first pospective follow-up study from a secondary referral centre. J Int Med 2001;249:41-6.市中の診療所から紹介された２次紹介病院からの報告である． 18才以上の158例について，平均追跡期間は22ヶ月．第一段階では，病歴，身体所見，一般臨床検査，胸のレントゲン，心電図，腹部エコー，甲状腺機 能，便潜血を施行した．これらの検査で診断が確定しなかった場合は，第一段階の検査結果を参考にして，上下消化管の内視鏡と，吸収不良症候群の診断を主目 的とした消化管の機能検査を行なった．その結果，132例（84％）で原因が判明したが，残りの26例（16％）では当初原因がわからなかった．

158例中，悪性疾患以外の身体疾患によるものが半数，悪性疾患が4分の一，1割が精神疾患によるものだった．当初原因不明だった26例のうち，追 跡期間中に原因が判明したのは7例で，そのうち，甲状腺機能亢進症，糖尿病，うつ病が各2例，故意の体重減少が1例だった．

○急性発症が多いと思いこんでいたのですが，急性発症はまれで週，月単位でゆっくり進行するとのことです．
○年齢も壮年にピークがあるように思いこんでいましたが，そういうことも書いてありません．ただし，封入体筋炎は明らかに高齢者に多いですから，その比較で言うと，高齢者では多発筋炎よりも封入体筋炎を考えるということになります．
○これも以外でしたが，腱反射は進行期を除いて保たれることが多いのだそうです．
○また，筋痛は３０％未満だとか．これも以外でした．
○封入体筋炎とは，CKの高さである程度区別がつく（封入体筋炎では１０００を越えることはほとんどない）というのが神経内科医の間でのコンセンサスだと思っていましたが，この総説にはそんなことは書いてありませんでした．
○Jo-1抗体の陽性率は，２０％前後だそうです．ですから，Jo-1抗体が陰性だからといって除外してはいけないわけです．

てんかんと聞くと、神経内科医でも若年者に圧倒的に多い病気のように思ってしまうが、実はそうではない。各年齢別に頻度のヒストグラムを描くと、U字型と いうより、むしろJ字型に近くなる。つまり、若年者でてんかんが最も多い4才以下から、年齢が上がるに従って頻度も下がるが、45才をボトムに頻度が再上 昇し、65才以降は4才以下の頻度を上回る率でてんかんが起こるという。これはもちろん、加齢に伴って脳卒中に代表される脳の器質的病変が増えるからなの だが、てんかんは小児や若者の病気という印象を訂正し、適切な診療を行うのに役立つ総説である。

この総説では，第二世代の向精神病薬を、錐体外路症状が少ないとして推奨しているが、最近のランセットのmeta-analysis (Lancet 2003; 361: 1581-89)によれば、必ずしも、第二世代が第一世代よりすぐれているとは限らない。一方、耐糖能に対する悪影響は第二世代の方が強い。今のところ、 第二世代が第一選択剤になるという強固なエビデンスはない。日本で認可されていないクロザピンだけは、標準的な治療に抵抗する場合に有効だが、お決まりの 無顆粒球症や心筋障害の副作用が問題になる。
