INFORMATION ON HOTEL RESERVATION FOR
The 69th Annual Meeting of the Japanese Orthodontic Society
Kinki Nippon Tourist Co., Ltd Global Business Management Branch (KNT) has been appointed as an official agent for the Meeting and will handle hotel accommodations. KNT has reserved a sufficient number of rooms at the following hotel for the participants during the Meeting period at special discount rates.
Hotel assignment will be made on a first-come, first-served basis.
The following rates are per person, per night, including breakfast and service charge and consumption tax.

- Reservation Procedure
Fill in the attached application form, and send it by fax to KNT.
- Payment
We send you confirmation slip and invoice until 30th Oct 2009.
After check the slip, please send charge by Bank Transfer till 6th November.
The bank transfer fee shall be borne by the participants.
Personal checks will not be accepted.
In order to make the hotel reservation, you will be asked for total amount.

- Cancellation / Revision
