Even though new orthodontic materials and tools have been constantly developed, the fundamental treatment modalities are almost the same as the ones from several decades ago. In this presentation, I will show the tissue-friendly treatment protocol using the thinnest NiTi such as .010 inch or .012 inch, lace-backs and stoppers. With the help of extra wire length (bulge excess), I can deliver the concentrated orthodontic force driven by the wire to the target area where crowding exists. With this technique, we can distribute the orthodontic force throughout the entire dentition, whereas the force becomes concentrated around the crowded area when we use conventional leveling technique. Many orthodontist are very familiar with the traditional concept of the “envelope of discrepancy", which describes the biological and physiological limitations of tooth movement. We also believe that the recent development of new treatment modalities including anchorage miniscrews, biomechanics, and orthodontic materials, have also allowed us to broaden our limitations. However, in reality, is this true? This RTD will specifically deal with the physiological and biologic limitations that may interfere tooth movement by reviewing clinical cases and 3D images and would like to propose modifications to the conventional “envelope of discrepancies".
