2. Participation certificates

(1) Please be sure to write your name and affiliation on a participation certificate and always display the certificate on your chest
　　 at the meeting venue. Those who do not have a participation certificate will not be allowed to enter the venue.

(2) Participation certificate cases are provided on a table at the general reception desk.

3. Pre-registered participants

(1) Pre-registered participants are not required to come to general reception for entry.
　　 Participation certificate cases are provided on a table at the general reception desk.

(2) Please be sure to bring this Program and Abstracts booklet with you, since the booklet will not be distributed
　　 to pre-registered participants at the venue.

4. On-site registered participants

(1) After filling out a registration application form, please submit the application form and pay the registration fee
　　at the on-site registration desk at the registration area and receive a participation certificate.
　　Program and Abstracts booklets will be distributed to non-member participants. Registration fees are as follows:

5. Sale of Program and Abstracts booklets

The booklets will be sold at 3,000 yen per volume at the venue.

6. Annual membership fees

If you have not paid the annual membership fee, please make a payment at the membership fee payment desk at
the general reception desk. The payment desk is open during the following hours:

7. IC cards for life training programs

To register points that you can earn for the Japan Dental Association’s life training programs by attending to this
Meeting, bring your IC-embedded membership card of the Association to the general reception desk because
your card information will need to be read with a card reader.

8. Training program point ID cards for JOS Accredited orthodontists

To register training points that you can earn for JOS orthodontist certification by attending this Meeting or its seminars,
bring your ID card to the training program point reception desk for JOS Accredited orthodontists because your
card information will need to be read with a card reader at the reception desk. Please note that you need to register
separately for the Life Education Seminar, the JOS Forum and the Annual Meeting.

9. Photographing and video recording

Photographing and video recording are not allowed in any lectures. Regarding academic exhibits, case exhibits
and case reports, photographing and video recording are permitted only for those indicated by a “Photography/
videotaping allowed” sign.

10. Simultaneous interpretation service

Simultaneous interpretation service will be provided for some lectures. A receiver rental service (free of
charge) is also available at the simultaneous interpretation receiver desk at the entrance of each lecture hall.
After using a receiver, please be sure to return it to the desk. A separate charge will be imposed for any radio
receivers not returned or lost, whatever the cause.

11. Cloakroom

A cloakroom is located in front of the general reception desk at the Atrium in Nagoya Congress Center.

12. Paging service

Paging/message services in the venue will be accepted at the information desk only in emergency. In all
other cases, use the message board for members, which will be provided next to the general information desk.

13. Parking

To avoid heavy traffic around the venue, please be advised not to come to the venue by car.

14. Accommodation information

Service desk of JTB Support Chubu Inc. is available at the registration area. Please feel free to consult with
the staff about accommodation, transportation, sightseeing, etc.

15. Consultation counter for orthodontic treatment under national health insurance
　　 (Medical Issues Examination Committee)

A consultation counter for orthodontic treatment under national health insurance is available at the space for case
exhibits for certification renewal of JOS expert orthodontists from 9:30 to 11:00 on October 20 (Thu.). Please feel
free to visit if you have any question or need assistance.

16. Consultation counter for certification of JOS Board-Certified orthodontists

A consultation counter for new applications and certification renewals of JOS Board-Certified orthodontists is available at
the space for case exhibits for certification renewal of JOS Board-Certified orthodontists from 10:00 to 12:00 on October 19(Wed.)
and from 10:00 to 12:00 on October 20 (Thu.).
