指導教官（助手以上）に確認署名（自筆）を記入していただき、在籍を確認した月の翌月1-10日（例えば5月の在籍確認は6月1-10日）に、この文書をFAXにて国際交流室（送付先5803-1817）に送信すること。あるいは、学内便にて、この文書を国際交流室に送付すること。在籍確認の手続きが期間内に行なわれない場合は、学習奨励費の支給は一時休止されるので注意すること。

Please submit this form with the signature of your supervisor (jo-shu or higher position) to the OIAA between the 1^st and the 10^th of month after the month for which you confirmed your enrollment by FAX or campus mail (FAX: 5803-1817). (For example, if the confirmation is for May, submit this form between June 1^st and June 10^th. Please be aware that Gakushu Shourei-hi scholarship payment will be stopped if the confirmation is not completed within the designated period.
