Requesting Permission from the International Heart Journal (Formerly Japanese Heart Journal) Association to Reproduce Entire Articles or Portions of Articles

When requesting permission to reproduce or duplicate a published article or any portion of an article that has been published in International Heart Journal (IHJ) or its predecessor Japanese Heart Journal (JHJ) and whose copyright is held by the International Heart Journal Association, the following will be required BEFORE any reproduction or duplication is allowed:

1) A completed Permission Form signed by the applicant should be sent by mail to the IHJ Association. The forms can be obtained as follows;

2) All material, including tables and figures, should be accurately reproduced and a proper citation should be clearly acknowledged and displayed in the duplicated article.

3) Two copies of ALL materials (articles, figures, tables, etc) to be duplicated should be sent to us by mail.

4) A royalty fee must be paid to us (no fee is required for academic uses). Please clearly state the intended purpose. We will determine the fee to be paid based on this information.

Duplication fee is to be calculated as follows: (price/issue: 3,500 yen) x {(number of pages) / (number of average pages per issue)} x (number of copies) x 0.05
Example: 3,500 x 2/ 72 x 1,000 x 0.05 = \ 4,861
